Use minimum necessary fields
Record the approved patient identifier, referral source, requested service as stated, received time, required administrative documents, destination, and current owner inside the authorized system.
Do not copy clinical detail into general chat or task tools. Use the documented urgent-message route when the source includes an emergency or clinical concern.
